Immunizations

If you have received a letter from Halton Region regarding your students incomplete immunization record and possible future school suspension:

Parents/guardians must complete the following steps:
  1. Review your child’s online immunization record at halton.ca/immunize to ensure all vaccines your child has received are included and add any missing vaccines to their online record.
  2. If your child has missed any required vaccines, make an immunization appointment with a health care provider.
  3. Report vaccines online at halton.ca/immunize. Every time a health care provider immunizes a student, parents/guardians must report the vaccines to Public Health. Health care providers do not report the vaccines they give to their patients.
Students must have up to date immunization records for the following ISPA designated diseases:
  • Tetanus, Diphtheria, Pertussis, Polio;
  • Measles, Mumps, Rubella; 
  • Meningococcal Disease; and
  • Varicella (chickenpox) (required for children born in 2010 or later).

 Do not submit any immunization documents to the school. We do not receive vaccination paperwork from familes ever. For more information: please visit www.halton.ca/immunize

Important Note:
Doctors do NOT notify Public Health
about your child's immunizations. So when your child receives their immunizations
YOU must notify Public Health through the ICON portal or by calling 311, every single time.

HDSB International Student Program Participants for Out of Ontario Student Vaccine Information to share with Halton Public Health

Students over 16 years, or parent/guardians/ custodians of students under 16 years, can either take their home country vaccine records in person to the Halton Regional Headquarters Building at 1151 Bronte Road in Oakville.

OR a digit copy/photo can be emailed to accesshalton@halton.ca

AND - request an Immunization Identification Number  and Online Account access by emailing vaccinerecords@halton.ca

For the email Subject Line, use -->  "Immunization Identification Number For HDSB Student Outside of Ontario Vaccination Record Submission"

In the email body, please attach:

 a photo or bof the student's vaccination record from their outside of Ontario jurisdiction, 

AND, include the following student information in the email:

Student full LEGAL Name, which would be the name on their passport/ birth certificate.

Student Date of Birth, written with Month Day, Year - For example January 1, 2021

The HDSB School the student(s) are attending - For example: Joshua Creek Public School, Burlington Central High School, or Thomas A. Blakelock High School. Please write the student’s school!

IF there is more than one student in the family, Halton Public Health MUST receive one email per student, with that student’s information. So, for two students in a family, that means two separate emails, one for each student with their own personal vaccine information.

Halton Region Public Health will confirm the student’s vaccine records and provide the student with a paper card/ record that can be kept for their proof of vaccination in Ontario, as well as create a student account with Halton Public Health.

Please register with a NON HDSB Email address (like @icloud.com, @gmail.com, or @qq.com) as the @hdsb.ca email address will be closed and inaccessible once the student graduates, or leaves the Halton District School Board. 

If students are missing any of the school vaccines, please also connect with StudyInsured Health Insurance at 1-866-883-9798 to have StudyInsured evaluate students vaccination records, and recommend which Doctor and Clinic to visit.

To access Healthcare, 

IF it is an Emergency (like a broken leg, loss of consciousness, or major bleeding) call 9-1-1 (go to the hospital.)

For minor ailments or medical checks, you will need to FIRST CALL StudyInsured Assistance at: 1-866-883-9787

When calling StudyInsured, ALSO have your StudyInsured Wallet Card ready to go, the phone staff will ask for your StudyInsured policy number.

Students, please check your HDSB email for information about your Wallet Card. Your LEGAL Guardian should also have a copy of your HDSB StudyInsured Wallet Card page. If you are under the age of 18 years, please do connect with your Legal Guardian for support in speaking with StudyInsured.
